Ages & Stages Questionnaire
(ASQ-3)
The Ages & Stages Questionnaire gives parents a quick look at how their children are doing in important areas such as communication, physical ability, social skills, and problem-solving skills. It will also help identify a child's strengths as well as areas where a child may need support.

Ages & Stages Social-Emotional Questionnaire
(ASQ:SE-2)
To better understand how your child is developing socially and emotionally, complete the Ages & Stages Social Emotional )ASQ-SE) Questionnaire. You will be asked questions about your child's ability to calm down, take direction, follow rules, communicate, perform daily activities, act independently, demonstrate feelings, and interact with others.
